Complete
the information on page 2 of the brochure as follows.
•In
the "instrument(s)" section, put your main
instrument first, and other instruments in descending
order of preference. We will try to place you
on your first instrument, but may ask you to attend
on one of your others if necessary.
•With
your completed application form, include an essay detailing
your interest in, and experience with, jazz. Tell us
why you want to attend camp, and also include a letter
of recommendation from someone who knows about your
musical ability and level of interest in traditional
jazz.
When
you are finished with your application and your essay,
please mail it by April 1, 2005 to:

Tuition
for the 2005 Sacramento Traditional Jazz Youth Camp
is $525.00 if paid by June 15, 2005; $575.00 if paid
after that date. Payment plans are available and many
jazz societies provide local scholarships. Please write The
